    I have trouble keeping my adapter in as well. I'm going to grind down the sides when I get a chance.

    Strange... mine has never came loose in my Makita, well over a year now with usage a few times per week. I think I tightened it with channel locks on the chuck key though

    I'm always taking mine in and out of the chuck. I have a 5 gallon pail with all my eel stuff in it. Blades, drill, etc...
    The drill won't fit into the bucket with the adapter in it, so I remove it when done. I think I'll cut the shaft down so it fits into the bucket with the adapter in the chuck. Then I'll tighten it with channel locks.
